2017 Hypertension Control Challenge Dear Partner in Blood Pressure Control, High blood pressure affects 1 in every 3 American adults, putting them at risk for heart disease and stroke. That’s why Million Hearts® launched the annual Hypertension Control Challenge in 2012—to recognize clinicians, practices, and health systems that achieve blood pressure control for their patients. Is your practice implementing evidence-based strategies to prevent and control hypertension? We want to hear from you! The 2017 Hypertension Control Challenge is now accepting nominations. Clinicians, practices, and health systems that provide continuing care and have achieved hypertension control rates greater than 70% are eligible to enter.  The submission deadline is June 2, 2017. A Champion in Action Reliant Medical Group is one recent Champion. Reliant’s health care team used evidence-based strategies to help at least 70% of patients with high blood pressure achieve control. The team standardized measurements, used digital readings, and gave away digital home blood pressure monitors to patients. Patients uploaded their data into electronic health records each week, helping the health care team track progress and identify patients with previously undiagnosed hypertension.
