Multi-, Routine-, & Non-Routine-Vaccine VISs

  • Multiple Vaccines (DTaP, Hib, Hepatitis B, Polio, and PCV13) (10/22/14)[Interim] UPDATED
This VIS may be used in place of the individual VISs for DTaP, Hib, Hepatitis B, Polio, and PCV13 when two or more of these vaccines are administered during the same visit. It may be used for infants through children receiving their routine 4-6 year vaccines.

VIS Ready in August

A new VIS for meningococcal B vaccines is due for release in August. Until the meningococcal B VIS is available, providers may use any means – verbal or written – to inform patients about the vaccine. Do not defer a vaccine dose because no VIS is available.

What are VISs?

Vaccine Information Statements (VISs) are information sheets produced by the CDC that explain both the benefits and risks of a vaccine to vaccine recipients.
Federal law requires that healthcare staff provide a VIS to a patient, parent, or legal representative before each dose of certain vaccines.
