Pulmonary embolism may present without chest pain and still be associated with markedly elevated cardiac biomarkers and right heart strain. The case also emphasizes the diagnostic value of echocardiography and CT angiography in identifying both the embolic burden and a right atrial thrombus. In hemodynamically stable patients, early anticoagulation may be sufficient to achieve complete thrombus resolution, highlighting the need for prompt recognition and treatment.
