Solid organ transplantation brings tremendous benefits to many patients with end-stage organ disease. However, challenges remain: a shortage of donated organs; the toxicities of immunosuppression medications; and inequitable access to the benefits of transplantation. This Lancet clinical Series describes promising opportunities to address these challenges. The first manuscript addresses technological advances that will increase the number of transplants including organ preservation, repair, and genetic engineering for xenotransplantation. The second manuscript describes advances in precision immunosuppression and multimodal approaches to diagnosing causes of allograft injury. The third and final manuscript summarises disparities in access to transplantation in diverse health systems across the globe, advocating for systems-level policies and interventions to equitable access to transplantation and patient-centred care.
