STEPS to Care, a new online toolkit to support engagement in HIV care, is now available for download on the Effective Interventions website. This multi-media package is designed to support service providers and agency personnel in the uptake and implementation of three evidence-informed linkage, retention and reengagement strategies, Patient Navigation, Care Team Coordination, and HIV Self-Management. The tool kit includes assessment, planning, tracking, logistics, and other easy to use forms and protocols. Modeled on the New York City Department of Health and Mental Hygiene (NYCDOHMH) Ryan White Care Coordination program, STEPS to Care was developed through a cooperative agreement between CDC, NYCDOHMH and EDC (Education Development Center.)
